Virginia Care Partners was established in 2012 in partnership with HCA Virginia and area physicians, both independent and HCA-employed to improve the delivery and quality of care and reduce the cost of care through a coordinated physician-driven network. Implementation of a standardized approach to gather and monitor clinical data based on quality measures across the continuum of the care setting (inpatient, outpatient and ambulatory) was essential.
By aligning the priorities of patients, payors, employers and providers in creating integrated prevention and care management services, VCP:

- Reduces the cost of care for everyone
A physician governed, physician led network, VCP is made up of more than 850 physicians and providers representing over 40 specialties including medical and surgical specialists, hospital-based physicians, behavioral health providers (PhD and PsyD) and midlevel providers.

VCP recognizes that practicing evidence-based medicine is the key to increasing quality and providing superior management of high cost conditions. Equally as important in building a high-value network is the ability to demonstrate data relative to provider performance. Crimson's Population Risk Management tool combines the capture of performance metrics and claims-based reporting and analytics to better manage populations. The increased transparency and insight into performance and cost data allows network physicians to focus patient by patient on providing care that helps them stay healthy and reduce overall cost of care.
